About The Liberty Dental Program
Liberty is a
discount plan. All general dentistry
procedures are approximately 30% off of
our regular fees and all specialists (endo,
perio, pedo, ortho, OS) are 25% off.
Coverage is effective immediately as
soon as the patient signs up. It is $100
a year for a single patient, $140 for a
husband and wife, and $165 for the
entire family living in a household.
Signing Up
After the patient
is given a brochure and signs up, the
form is then emailed or faxed to Mike at
Liberty (email:
MichaelM@dentalgrp.com
/ fax: 732-914-8267). An insurance card
should be mailed within 30 days after
the patient signs up, so make sure their
address is correct.
Payments
When taking
payments, remember that it is to be
billed to corporate, not your
individual office. If a patient signs up
via credit card, write the card number
down when emailing or faxing to Liberty.
If the payment is cash or check, it is
sent to corporate via the corporate
envelope. Checks are made payable to
Liberty Dental Program.
Verifying Liberty
Liberty
eligibility should be verified once
every time the patient comes in.
